This looks great, well done!! The rounded corners are a nice touch that fit the feeling of the page well. Love that background image too!
Here’s a small suggestion, if you want: the spacing between different questions looks pretty similar to the spacing between each input and its label, especially in the section with radio buttons and checkboxes. I think it would help make the form easier to read if you made each label sit closer to its input field, or added more space separating the different questions from each other. (For example, I tried to do this in my form by making two classes called “question” and “answer” and with different amounts of margin)
